Medications can be an essential part of treatment for various diseases and conditions. It’s important to recognize how certain drugs may impact other areas of your health — including the pelvic floor. They can affect your pelvic floor health and rehabilitation outcomes in ways you might not expect. A commonly prescribed stimulant, Adderall, can cause pelvic health issues such as:
- Myofascial dysfunction – tension and pain in muscles including those of the pelvic floor,
- altered bladder and bowel function – issues with urination and digestion,
- hormonal imbalances – changes in hormone levels affecting pelvic health,
- neuromuscular function -affecting how your brain communicates with pelvic muscles.
The Effects of Adderall on Your Pelvic Health
Adderall is a stimulant medication commonly used to treat ADHD. It contains dextroamphetamine and amphetamine, which stimulate the central nervous system (CNS). This leads to a more active body-state, known as the “fight or flight” response. While it might boost focus and alertness, the physiological changes triggered by Adderall can have a negative impact on the pelvic floor.
Sympathetic Nervous System Activation
Adderall increases your body’s sympathetic nervous system (SNS) response, which:
- increases heart rate,
- raises blood pressure,
- dilates airways,
- redirects blood flow to muscles.
While this is helpful for staying alert and active, it creates a tense, overactive physical state. The pelvic floor muscles, like other skeletal muscles, respond to this heightened sympathetic tone. As a result, they may become tense or overactive leading to:
- pelvic floor dysfunction,
- painful intercourse (dyspareunia),
- difficulty relaxing during urination and bowel movements.
Effects on Bladder and Bowel Function
One of the most common side effects of Adderall is constipation. Here’s why—the body, in a “fight or flight” state, prioritizes essential functions like muscle activity and alertness, and redirects resources away from non-critical systems, like digestion. This can cause:
- slowed peristalsis (intestinal movement) leading to constipation,
- reduced appetite, contributing to digestive slow-down,
- diversion of blood flow away from the digestive system.
When you’re constipated, it can lead to pelvic dysfunction or tightness from repeated strain of attempting to pass stool. This may exacerbate pelvic pain or muscle tension. To manage these side effects:
- stay hydrated
- increase fiber intake,
- maintain proper bowel mechanics.
Did you know that your gut and pelvic floor communicate? Research has shown that changes in gut motility directly affect the tone and coordination of the pelvic floor muscles. So, when stimulant medications like Adderall slow down your gut, it can increase pelvic floor tension and pelvic muscle coordination issues.
Stress and Muscle Tension
Adderall commonly causes mood swings, anxiety, and irritability which can contribute to muscle tension throughout the body. This heightened stress response can lead to jaw clenching, muscle spasms, and body-wide tension. And these may result in pelvic pain, difficulty relaxing during urination, bowel movement, or sex. Relaxation techniques, breathwork and manual therapy are excellent ways to alleviate this tension and improve pelvic floor function.
Erectile and Orgasmic Dysfunction in Men
For men taking Adderall, there’s evidence to suggest that Adderall can impact sexual health, particularly when it comes to erectile dysfunction (ED) and orgasmic function. Adderall’s stimulating effects cause blood vessel constriction and increased sympathetic tone, which can reduce blood flow to the genitals making it more difficult to achieve an erection, or lead to delayed ejaculation and other sexual challenges. Studies have also shown that Adderall can affect semen parameters, highlighting the importance of understanding all potential side effects on reproductive health.
Sexual Arousal and Libido in Women
For women, Adderall can impact sexual arousal in a variety of ways. While the drug increases dopamine, which may enhance libido in some people, it also intensifies sympathetic nervous system activity. This can make arousal and orgasm more challenging due to increased muscle tension. It may make it difficult to relax during sexual activity. Moreover, the hormonal changes caused by Adderall can cause vaginal dryness, leading to painful intercourse (dyspareunia). Managing these symptoms with pelvic floor therapy and relaxation techniques can be incredibly helpful.
The Side Effects of Adderall Can Be Managed
